Principle Duties and Responsibilities

    • Identifies, tracks and manages care for high-risk patients in collaboration with the entire care team.  High-risk patients include individuals or families with co-morbidities such as diabetes, HTN, asthma and/or psychosocial issues.                     
    • Conducts initial care coordination screening eligibility and performs comprehensive risk assessment and medication reconciliation.
    • Oversees the development and implementation of an integrated patient care plan for each of the highest risk patients.
    • Responsible for achieving Care Management metrics including: transitions of care after admission, timely completion of care plans, decrease in hospital overutilization.
    • Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. 
    • Responsible for triage of patients who present for urgent care in the health center.
    • Assists with competency activities for Medical Assistants to ensure appropriate level of clinical skills and knowledge.
    • Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care, with attention to population based measures. 
    • Participates in center-wide training events and health related activities.
    • Participates in peer review chart audits. 
    • Responsible for meeting the established standards of his/her profession and for practicing within the scope of licensure. 
    • Performs all other duties as assigned

    Nurse Manager

     The Nurse Manager assists the National Kidney Foundation of Illinois in reaching its goal of improving the health and well-being of people at risk for or affected by kidney disease through prevention, education and empowerment.

    The Nurse Manager oversees the logistics and execution of KidneyMobile® screenings throughout the state of Illinois. This includes:
    - Keeping inventory of and ordering all KidneyMobile® medical supplies

    The Nurse Manager is also responsible for three patient programs throughout the year. These include: Living with Kidney Disease – Chicago, Living with Kidney Disease – Peoria and Kidney Camp.
    Living with Kidney Disease is a day-long patient education program offered in the spring and fall. The Nurse Manager is responsible for all planning, coordination, and execution of these events.
    Kidney Camp takes place in July at YMCA Camp Duncan in Ingleside, IL, and is a camp for children on dialysis or who have had a kidney transplant. The Nurse Manager is responsible for recruiting and communicating with campers and their families, and attends the week-long camp to assist medical staff with the needs of campers.
    Requirements: The Nurse Manager must have a BSN RN and be licensed (or eligible for licensure) in Illinois.
    Three to five years of experience in the renal field with good understanding of the connection between diabetes, hypertension and kidney disease.
    Must be willing to travel. The KidneyMobile® spends nine months per year traveling throughout the state and the greater Chicagoland area. Some evenings or weekends for screenings or other NKFI events are required.
